I know that methadone requires a specific (10, 11 or 12 panel) test, but I was curious about the lab testing. - Does the employer have to pay extra for this test, or will it show up in the regular lab testing that they do?

Most likely not. Common 5 and 6 panel urine screens generally test for opiates, cocaine, benzodiazepines, barbiturates, amphetamines, methamphetamine, THC and maybe alcohol. The test for methadone is expensive compared to others and methadone is rarely a drug of choice. Most employers don't check for it. If your getting methadone from a clinic, they can provide you with a Rx for it that most employers will accept. Even though it's medicine that keeps people from dying from overdose, spending their lives in jail and spending every dime on illegally buying pills that can cost hundreds a day, methadone has a negative stigma due to ignorance. If they don't ask, don't tell. It's no ones business but yours and your protected by federal law (assuming you're in a treatment program). If you take methadone off the street, you're probably overdose and die waiting for a buzz due to the half-life. Don't play with it!
